In this article, you can read the instructions for using the drug Es Corinth. There are reviews of visitors to the site - consumers of this medication, as well as opinions of doctors of specialists on the use of EsCordi Cora in their practice. A big request is to actively add their feedback on the drug: the medicine helped or did not help get rid of the disease, which were observed complications and side effects, possibly not declared by the manufacturer in the annotation. Analogs of EsCord Core with available structural analogues. Use for the treatment of hypertension or hypertension in adults, children, as well as in pregnancy and lactation. Composition of the preparation.

Es Cori - a slow calcium channel blocker (BCCC), a dihydropyridine derivative, an S-negative isomer of amlodipine. Has a more pronounced pharmacological action than R-positive amlodipine. It blocks Ca2 + channels, inhibits the transmembrane transition of Ca2 + into the cell (mostly in the smooth muscle cells of the vessels, rather than in the cardiomyocytes). Has an antianginal effect, as well as a prolonged dose-dependent hypotensive effect. A single dose provides a clinically significant reduction in blood pressure (BP) 2-4 hours after ingestion, which persists for 24 hours (lying and standing).

Composition

Levamlodipine + excipients.

Pharmacokinetics

Absorption of levlodipine in the gastrointestinal tract (GIT) does not change when eating. Bioavailability - 65%; has the effect of a "first pass" through the liver. The connection with proteins is 93%. Most of the drug is distributed in the tissues, the smaller part is in the blood. Penetrates through the blood-brain barrier (BBB). Metabolism is 90% in the liver (slow, but extensive) with the formation of inactive metabolites. It is excreted by the kidneys (60% - in the form of metabolites, 10% - unchanged), intestines (20-25%), and breast milk. When hemodialysis is not removed.

Application in pregnancy and lactation

Contraindicated the use of EsCordi Cora during pregnancy and lactation.

Use in children

Contraindicated in children and adolescents under the age of 18 years.

Application in elderly patients

In elderly patients, the elimination half-life and clearance of the drug are prolonged, therefore, when the dose is increased, careful monitoring is necessary.

special instructions

During the treatment it is necessary to control body weight, intake of Na (the appropriate diet), observe the hygiene of the teeth, visit the dentist (to prevent soreness, hemorrhage and hyperplasia of the gingival mucosa).

Despite the absence of withdrawal syndrome, a gradual reduction of doses is recommended before cessation of treatment.

Caution should be applied to EsCord Cor in the syndrome of weakness of the sinus node, chronic heart failure of non-ischemic etiology in the stage of decompensation, moderate arterial hypotension, aortic and mitral stenoses, hypertrophic cardiomyopathy, myocardial infarction (and within 1 month after it), diabetes mellitus lipid metabolism, liver failure, as well as in the elderly.

Impact on the ability to drive vehicles and manage mechanisms

During the period of treatment by EsCord Corom, care must be taken when driving vehicles and engaging in potentially dangerous activities requiring increased attention and speed of psychomotor reactions.

Drug Interactions

Inhibitors of microsomal oxidation increase the concentration of the drug in the blood plasma, increasing the risk of side effects, and inducers of microsomal enzymes of the liver - reduce.

Alpha-adrenostimulants, estrogens (Na delay), sympathomimetics weaken the hypotensive effect.

Thiazide and loop diuretics, beta-adrenoblockers, verapamil, nitrates increase the anti-anginal and hypotensive effects of EsCordi Kor.

Amiodarone, quinidine, alpha-adrenoblockers, antipsychotics, BCCI may increase the hypotensive effect.

Lithium preparations - the risk of increasing neurotoxicity (nausea, vomiting, diarrhea, ataxia, tremor, tinnitus).

Calcium preparations can reduce the effect of BCCC.

Procainamide, quinidine and other drugs that cause prolongation of the QT interval, increase the negative inotropic effect and increase the risk of a significant prolongation of the QT interval.

Response of a cardiologist

I have patients with arterial hypertension of various origins. Most of them are patients with primary (essential hypertension). The drug EsCord Cor, I appoint this category of patients. I start, as a rule, from the appointment of 2.5 mg per day.At a part of patients the arterial pressure decreases and stabilizes already at this dose. Others have to increase the dose to 5 mg per day. If this is not enough, I select another drug. Side effects on the therapy of EsCord Korom happen. Among them: insomnia or, on the contrary, drowsiness, swelling of the lower extremities, nausea.
