In this article, you can read the instructions for using the drug Fluimucil. Comments of visitors of the site - consumers of this medication, as well as opinions of doctors of specialists on the use of Fluimucil in their practice are presented. A big request is to actively add their feedback on the drug: the medicine helped or did not help get rid of the disease, which were observed complications and side effects, possibly not declared by the manufacturer in the annotation. Analogues of Fluimucil in the presence of existing structural analogues.Use for treatment of cough and thinning of sputum in adults, children, as well as during pregnancy and lactation.
Fluimucil mucolytic drug. It dilutes sputum, increases its volume and facilitates its separation. Action Acetylcysteine (active ingredient Fluimucil drug) is associated with the ability of the sulfhydryl groups to break intermolecular and intramolecular disulfide bonds, acid mucopolysaccharides mucus, which leads to depolarization and mukoproteidov decrease sputum viscosity. Preserves activity in the presence of purulent sputum.
Increases the secretion of less viscous sialomucins by goblet cells, reduces the adhesion of bacteria to the epithelial cells of the bronchial mucosa. Stimulates mucous cells of bronchi, the secret of which lyses fibrin. A similar effect has on the secret formed by inflammatory diseases of the ENT organs.
It has an antioxidant effect due to the presence of an SH-group capable of neutralizing electrophilic oxidative toxins.
Acetylcysteine easily penetrates the cell, deacetylates to L-cysteine, from which intracellular glutathione is synthesized.Glutathione is a highly reactive tripeptide, a potent antioxidant and cytoprotector that neutralizes endogenous and exogenous free radicals and toxins. Acetylcysteine prevents depletion and promotes an increase in the synthesis of intracellular glutathione involved in redox processes of cells, contributing to the detoxification of harmful substances. This explains the effect of acetylcysteine as an antidote in Paracetamol poisoning.
Protects alpha 1 antitrypsin (elastase inhibitor) from the inactivating effect of the HOCl oxidant produced by myeloperoxidase active phagocytes. It also has an anti-inflammatory effect (by suppressing the formation of free radicals and active oxygen-containing substances responsible for the development of inflammation in the lung tissue).
Composition
Acetylcysteine + excipients.
Thiamphenicol glycinate acetylcysteinate + excipients (Fluimucil antibiotic IT).
Pharmacokinetics
When ingestion is well absorbed from the digestive tract. Penetrates into the intercellular space, mainly distributed in the liver, kidneys, lungs, bronchial secretions. Penetrates through the placental barrier.Rapidly deacetylated to cysteine in the liver. In plasma, a mobile equilibrium of free and protein-bound plasma acetylcysteine and its metabolites (cysteine, cystine, diacetylcystine) is observed. It is excreted in the urine in the form of inactive metabolites (inorganic sulfates, diacetylcysteine), a small part is excreted unchanged through the intestine.
Indications
- respiratory diseases accompanied by sputum discharge (including bronchitis, tracheitis, bronchiolitis, pneumonia, bronchiectasis, cystic fibrosis, lung abscess, pulmonary emphysema, laryngotracheitis, interstitial lung diseases, atelectasis of the lung (due to congestion of the bronchial mucosa);
- catarrhal and purulent otitis, sinusitis, incl. sinusitis (to ease the secretion);
- to remove the viscous secretion from the respiratory tract in post-traumatic and postoperative conditions;
- preparation for bronchoscopy, bronchography, aspiration drainage;
- for washing abscesses, nasal passages, maxillary sinuses, middle ear, fistula treatment, operating field for operations on the nasal cavity and mastoid process;
- with concomitant nonspecific forms of respiratory infections to improve drainage, including cavernous foci, with mycobacterial infections.
Forms of release
Solution for injection and inhalation (sometimes mistakenly called syrup).
The effervescent tablets are 600 mg.
Granules for solution for oral administration 200 mg (sometimes mistakenly called powder).
Lyophilizate for the preparation of solution for injection and inhalation in ampoules (Fluimucil antibiotic IT).
Instructions for use and how to use them
Solution
Inhalation
For aerosol therapy in ultrasound devices, spray 3-9 ml of 10% solution, in devices with a control valve - 6 ml of 10% solution. The duration of inhalation is 15-20 min; multiplicity - 2-4 times a day. In the treatment of acute conditions, the average duration of therapy is 5-10 days; with prolonged therapy of chronic conditions treatment course - up to 6 months.
Due to the high safety of the drug, the frequency of taking and the relative amount of the dose can be changed by the doctor within the permissible limits, depending on the patient's condition and the therapeutic effect. There is no need to change the adult dose in the case of using the drug to treat children.In the case of a strong secretolytic action, the secretion is aspirated, and the frequency of inhalations and the daily dose are reduced.
The therapeutic mixture should not be placed in an ultrasonic nebulizer, because it quickly destroys the active substance of Fluimucil. It is desirable to use a compressor device for inhalation, which performs an ideal fitting of the sizes of the inhaled particles of the preparation.
Intratracheal
For washing the bronchial tree with therapeutic bronchoscopy, 1-2 ampoules or more per day are used depending on the clinical indications.
Locally
Bury in the external ear canal and nasal passages (in the nose) 150-300 mg (1.5-3 ml) per 1 procedure.
Parenterally
Enter intravenously (preferably drip or slowly struino for 5 minutes) or intramuscularly. Adults - 300 mg (3 ml) 1-2 times a day; children from 6 to 14 years - 150 mg (1.5 ml) 1-2 times a day. The daily dose for children under 6 years - 10 mg / kg of body weight, children under the age of 1 year in / in the introduction of acetylcysteine is possible only for vital indications in a hospital. The duration of treatment should be determined by the results of a change in the patient's condition.
It should be borne in mind that children under the age of 6 years are preferably taking the drug orally.
High local and general tolerability of the drug allows for long-term treatment.
For intravenous administration, the solution is further diluted with 0.9% sodium chloride solution or 5% Dextrose solution in a 1: 1 ratio. The duration of therapy is determined individually (no more than 10 days).
In patients older than 65 years, use the lowest effective dose.
Effervescent tablets
The drug in the form of effervescent tablets is prescribed for adults to 600 mg (1 tablet) once a day. Before use, the effervescent tablet is dissolved in 1/3 cup of water.
Granules
Granules for the preparation of solution for oral administration are prescribed for adults and children over 6 years - 200 mg 2-3 times a day; children aged 2 to 6 years - 200 mg 2 times a day or 100 mg 3 times a day; children aged 1 year to 2 years - 100 mg 2 times a day.
In newborn children, the drug is used only for vital indications at a dose of 10 mg / kg of body weight under the strict supervision of a doctor.
Before use, the required amount of granules is dissolved in 1/3 cup water. Children of the first year of life are allowed to drink the resulting solution from a spoon or a bottle for feeding.
The duration of the course of treatment is set individually.In acute diseases, the duration of treatment is from 5 to 10 days, with chronic - up to several months.

Application in pregnancy and lactation
In pregnancy, the use of Fluimucil is possible only if the intended benefit to the mother exceeds the potential risk to the fetus.
If it is necessary to prescribe the drug during lactation, breastfeeding should be stopped.
special instructions
Patients with bronchial asthma and obstructive bronchitis acetylcysteine should be administered with caution under the systematic control of bronchial patency.
With a shallow intramuscular introduction of the drug and in the presence of hypersensitivity, there may appear a light and rapidly burning sensation, and therefore it is recommended to inject the drug deep into the muscle.
Ampoule Fluimucil is opened before use. The opened ampoule can be stored in the refrigerator for 24 hours, during this time the drug can be used only for inhalations.
The Fluimutsil solution should not come in contact with rubber and metal surfaces.
With caution appoint with gastric ulcer and duodenal ulcer in the phase of remission, varicose veins of the esophagus, hemoptysis, pulmonary hemorrhage, bronchial asthma (with intravenous injection increases the risk of bronchospasm), adrenal diseases, hepatic and / or kidney failure, hypertension, and also children under the age of 6 years.
Children under 1 year of intravenous injection of acetylcysteine is possible only for vital indications in a hospital.
Drug Interactions
Simultaneous use of Fluimucil with antitussive agents may increase sputum stasis due to suppression of the cough reflex.
While the use of antibiotics such as tetracyclines (doxycycline excluding), ampicillin, amphotericin B, possibly their interaction with a thiol group acetylcysteine, which leads to reduced activity of both drugs, so the interval between reception acetylcysteine and antibiotics should be at least 2 hours.
Simultaneous reception of Fluimucil and Nitroglycerin may lead to an increase in the vasodilator effect of the latter.
Acetylcysteine reduces the hepatotoxic effect of paracetamol.
Pharmaceutically incompatible with other drug solutions.
