Doxepin - an antidepressant of a tricyclic structure, a derivative of dibenzoxepine. The mechanism of action, apparently, is associated with the effect on adrenergic transmission in the central nervous system (CNS), in particular, with the blockade of neuronal seizure of noradrenaline. It also has anxiolytic, sedative activity, has m-cholinoblock effect. Does not have a stimulating effect on the central nervous system. Does not inhibit monoamine oxidase (MAO). Initially, the anxiolytic effect of doxepin (the active substance of the drug Doxepin) manifests itself, the optimal antidepressant effect develops after 2-3 weeks of treatment.

Composition

Doxepin + auxiliary substances.

Pharmacokinetics

Quickly and completely absorbed in the gastrointestinal tract (GIT). The maximum concentration of Doxepin in the blood is recorded after 1-3 hours. It penetrates the blood-brain barrier (GEB) and the placental barrier, into the milk of the nursing mother. Demethylated in the liver to demethylidosepin. It is excreted in the urine in the form of metabolites.

With a significant depression and / or anxiety, the effective dose can reach 300 mg per day. In patients with minimal disturbances, smaller doses of 25-50 mg per day may be effective.

Doxepin is taken orally 1-2 times a day (with a single dose during the day, the dose of doxepin should not exceed 150 mg).In sleep disorders, most of the daily dose is taken in the evening.

The dose for children over 12 years is 0.5 mg per 1 kg of body weight per day.

Impact on the ability to drive vehicles and manage mechanisms

In the treatment of Doxepin, special care should be taken when driving a car, servicing machinery, and performing work that requires increased concentration.

Drug Interactions

With simultaneous use with anticholinergics, anticholinergic effects may be enhanced.

With the simultaneous use of Doxexin in doses of more than 200-250 mg, it is possible to reduce or completely inhibit the antihypertensive action of guanethidine or preparations with a similar mechanism of action.

With simultaneous use with dextropropoxyphene, the oppressive effect on the CNS may be increased; with lithium carbonate - possible manifestations of neurotoxicity.

In patients with diabetes mellitus, when combined with doxepin and tolazamide, development of severe hypoglycemia is possible.

With simultaneous application with chlorpromazine, hyperpyrexia can develop.

With simultaneous use with cimetidine, an increase in the concentration of doxepin in the blood plasma, an increase in its main effect, as well as an anticholinergic side effect.

With simultaneous use with ethanol (alcohol), concentration of attention is disturbed and the speed of psychomotor reactions decreases.

Review of the neurologist's doctor

The drug Doxepine well relieves depressive and anxious states in elderly patients with pre-acute and senile psychoses. Usually, it is enough to prescribe a medicine for 1 capsule of 25 mg three times a day. There were in my practice side reactions to Doxepin therapy, manifested by drowsiness, dyspnoea, dizziness. In addition, I prescribe this drug as part of complex therapy for patients with chronic alcoholism for the management of depressive episodes and mild delusional syndromes.
