Analogues of Loxon antibiotic in the presence of existing structural analogues. Use for the treatment of otitis, cystitis, blepharitis and other infectious and inflammatory diseases of various genesis in adults, children, as well as during pregnancy and lactation. Composition of the preparation.

Loxon - antimicrobial synthetic agent of a group of fluoroquinolones of a wide spectrum of action. Has a bactericidal effect. Suppressing DNA-gyuraz (enzyme of pathogenic bacteria), violates the process of supercoiling of deoxyribonucleic acid (DNA). Highly active against most Gram-negative bacteria: Escherichia coli (E. coli), Salmonella spp. (salmonella), Shigella spp. (Shigella), Proteus spp. (Proteus), Morganella morganii, Klebsiella spp. (including Klebsiellapneumoniae), Enterobacterspp., Serratiaspp., Citrobacterspp., Yersiniaspp., Providenciaspp., Haemophilusinfluenzae, Pseudomonasaeruginosa, Neisseriagonorrhoeae, Neisseriameningitidis. It is active against some Gram-positive bacteria (including Staphylococcus aureus).

To Norfloxacin (active ingredient of the drug Lokson) are resistant to anaerobic bacteria, insensitive to Enterococcus spp. and Acinetobacter spp.

Composition

Norfloxacin + excipients.

Pharmacokinetics

After ingestion, the drug quickly, but not completely (30-40%) is absorbed from the digestive tract, eating reduces the rate of absorption. The maximum concentrations in the serum are observed after 1-2 hours. Binding to plasma proteins is 14%.

Up to 67% of norfloxacin penetrates from the blood plasma into the peripheral tissues. Concentrations exceeding plasma are noted in the bile duct, gall bladder, liver, kidney and prostate gland.The drug does not penetrate the GEB (blood-brain barrier), it penetrates the placental barrier. About 80% is excreted by the kidneys, 20% is excreted unchanged through the intestine.

Indications

Infectious-inflammatory diseases caused by microorganisms sensitive to norfloxacin.

For oral administration, they are used for such infectious-inflammatory pathologies:

  • diseases of the urinary tract;
  • diseases of the prostate;
  • pathology of the digestive tract;
  • gonorrhea;
  • prevention of recurrence of urinary tract infections, bacterial infections in patients with granulocytopenia, travelers' diarrhea.

For topical application:

  • conjunctivitis;
  • keratitis;
  • keratoconjunctivitis;
  • corneal ulcers;
  • blepharitis;
  • blepharoconjunctivitis;
  • acute inflammation of the meibomian glands;
  • otitis externa, acute otitis media, chronic otitis media;
  • dacryocystitis;
  • prevention of eye infections after removal of a foreign body from the cornea or conjunctiva, after damage by chemical agents, before and after surgical interventions on the eyes;
  • prevention of infectious complications in surgical interventions on the hearing organ.

Forms of release

The tablets covered with a cover of 400 mg.

Instructions for use and dosing regimen

The loxon is taken orally on an empty stomach (at least 1 hour before or 2 hours after eating) and drink plenty of fluids:

  • In acute uncomplicated cystitis appoint 200-400 mg 2 times a day for 3-5 days.
  • In acute infections of the urinary tract - 400 mg 2 times a day for 7-10 days.
  • With recurrent or chronic recurrent urinary tract infections - 400 mg twice a day for 4 weeks and 400 mg once a day for up to 12 weeks in case of a relapse.
  • For genital infections, 400-600 mg twice a day for 7 days.
  • In acute uncomplicated gonococcal infection - appoint 800 mg once.
  • For the prevention of sepsis in patients with neutropenia - 400 mg 3 times a day for 8 weeks.
  • With infections of the gastrointestinal tract - 400 mg 2 times a day for 3-5 days.
  • To prevent bacterial diarrhea in epidemically unfavorable areas - 400 mg once a day.
  • Patients with hepatic insufficiency and creatinine clearance of 10-30 ml per minute are prescribed 400 mg once a day.

In ophthalmology and ENT-practice apply topically.

Side effect

  • nausea;
  • heartburn;
  • anorexia (pathological desire to lose weight);
  • diarrhea;
  • abdominal pain;
  • headache;
  • dizziness;
  • feeling tired;
  • sleep disorders;
  • irritability;
  • sense of anxiety;
  • anxiety;
  • confusion of consciousness;
  • hallucinations;
  • depression;
  • noise in ears;
  • skin rash;
  • itching;
  • Quincke's edema (extensive swelling of the skin, subcutaneous tissue, fascia, muscle);
  • interstitial nephritis (acute or chronic inflammatory process in the interstitial tissue and tubules of the kidneys);
  • tendonitis (inflammation and dystrophy of the tendon tissue), tendon rupture.

Contraindications

  • pregnancy;
  • lactation (breastfeeding);
  • children and adolescents under 18;
  • increased sensitivity to norfloxacin or other drugs from the group of fluoroquinolones.

Application in pregnancy and lactation

Lokson contraindicated in pregnancy and lactation (breastfeeding), because in experimental studies found that it causes arthropathy (secondary damage to the joints). If it is necessary to prescribe norfloxacin during lactation, the question of stopping breastfeeding should be solved.

Use in children

Contraindicated in childhood and adolescence (up to 15 years).

special instructions

It should be used with caution in patients with epilepsy, convulsive syndrome of another etiology, with severe impairment of kidney and liver function.During treatment, patients should receive a sufficient amount of fluid (under the control of diuresis).

During the treatment with norfloxacin, an increase in the prothrombin index was noted. When performing surgical interventions, the condition of the blood coagulation system should be monitored.

During treatment with Loxon, exposure to direct sunlight should be avoided.

When pain occurs in the tendons or at the first signs of tendovaginitis, it is recommended to cancel the drug.

Norfloxacin should be taken at least 2 hours before or 2 hours after taking antacid agents or preparations containing iron, zinc, magnesium, calcium or sucralfate.

Impact on the ability to drive vehicles and manage mechanisms

In connection with the possible occurrence of dizziness, patients taking norfloxacin should be careful when driving a car and doing other potentially dangerous activities that require increased attention and speed of psychomotor reactions.

Drug Interactions

With the simultaneous use of norfloxacin with warfarin, the anticoagulant effect of the latter is enhanced.

With the simultaneous use of norfloxacin with cyclosporine, there is an increase in the concentration of the latter in the blood plasma.

With the simultaneous administration of Loxon and antacid agents or preparations containing iron, zinc, magnesium, calcium or sucralfate, the absorption of norfloxacin is reduced due to the formation of complexons with metal ions (the interval between their intake should be at least 4 hours).

With the simultaneous administration of norfloxacin reduces the clearance of theophylline by 25%, so with simultaneous use should reduce the dose of theophylline.

Simultaneous administration of Loxon with drugs that have the potential to reduce blood pressure (arterial pressure) can cause a sharp decrease in it. In this regard, in such cases, as well as with the simultaneous introduction of barbiturates, anesthetics, it is necessary to monitor heart rate (heart rate), blood pressure, ECG. Simultaneous use with drugs that reduce the epileptic threshold may lead to the development of epileptiform seizures.

Reduces the effect of nitrofurans.
