In this article, you can read the instructions for using the drug Bicillin. There are reviews of visitors to the site - consumers of this medication, as well as opinions of doctors specialists on the use of antibiotics Bicillin in their practice. A big request is to actively add their feedback on the drug: the medicine helped or did not help get rid of the disease, which were observed complications and side effects, possibly not declared by the manufacturer in the annotation. Analogues of Bicillin in the presence of existing structural analogues.Use for the treatment of syphilis, scarlet fever, erysipelas and other infections in adults, children, as well as during pregnancy and lactation.

Bicillin - antibiotic group of biosynthetic penicillins. The mechanism of antimicrobial action is based on the disruption of the synthesis of peptidoglycan - mucopeptide of the cell membrane, which leads to inhibition of the synthesis of the cell wall of the microorganism, suppression of growth and reproduction of bacteria.

It is active against gram-positive bacteria, gram-negative cocci, Actinomyces spp., Spirochaetaceae.

Inactive with strains of Staphylococcus spp. (staphylococcus) producing penicillinase.

Has a prolonged effect.

Bicillin 3

Combined antibacterial preparation of a group of penicillins of prolonged action, which are destroyed by penicillinase. At the heart of the mechanism of action is the disruption of the synthesis of peptidoglycan - mucopeptide of the cell membrane, which leads to inhibition of the synthesis of the cell wall of the microorganism, suppression of growth and reproduction of bacteria.

It is active against gram-positive bacteria: Staphylococcus spp. (not forming penicillinase), Streptococcus spp. (streptococcus) (incl.Streptococcus pneumoniae), Corynebacterium diphtheriae, Bacillus anthracis; Gram-negative bacteria: Neisseria gonorrhoeae, Neisseria meningitidis, Actinomyces israelii.

The drug is also active against Treponema spp., Anaerobic spore-forming rods, leishmanias.

Staphylococcus spp., Which produce penicillinase, are resistant to the drug.

Bicillin 5

Combined bactericidal antibiotic, consisting of two long-acting benzylpenicillin salts.

Composition

Benzathine benzylpenicillin + excipients (Bicillin 1).

Benzathine benzylpenicillin + Benzylpenicillin sodium salt + Benzylpenicillin novocaine salt + excipients (Bicillin 3).

Benzathine benzylpenicillin + Benzylpenicillin novocaine salt (benzylpenicillin procaine) + excipients (Bicillin 5).

Pharmacokinetics

Bicillin 1

When intramuscular injection is slowly absorbed from the injection site, providing a prolonged effect. Poorly penetrates the blood-brain barrier (BBB). Metabolized by hydrolysis to benzylpenicillin. Due to prolonged absorption, benzylpenicillin is detected in the urine within 12 weeks after the administration of a single dose.

Bicillin 3

With a single administration, it remains in the average therapeutic concentration for 6-7 days. After intramuscular injection slowly hydrolyzed with the release of benzylpenicillin. To an insignificant degree is subjected to biotransformation. Binding to blood plasma proteins - 40-60%. Passes through the placental barrier, penetrates into breast milk. Well distributed in liquids, with distribution in tissues - low. It is excreted mainly by the kidneys.

Bicillin 5

It is a drug of prolonged action, a high concentration of antibiotic in the blood is stored up to 4 weeks. After intramuscular injection of benzathine, benzylpenicillin very slowly hydrolyzes, releasing benzylpenicillin. The maximum concentration of the drug in the blood serum is reached 12-24 hours after the injection. Diffusion of the drug in the liquid is complete, the diffusion in the tissue is very weak. Connection with plasma proteins 40-60%. Benzathine benzylpenicillin passes in small amounts through the placental barrier, and also penetrates into the mother's breast milk. Biotransformation of the drug is insignificant. It is excreted mainly by the kidneys in an unchanged form.

Indications

Rules for the preparation, dilution and administration of a solution Bicillin 3

A suspension of Bicillin 3 is prepared immediately prior to use. In the bottle with the drug injected with 2-3 ml of sterile water for injection or isotonic sodium chloride solution. The mixture in the vial is shaken for 30 seconds (in the direction of the longitudinal axis) until a homogeneous suspension (or slurry) is formed, which is injected deeply intramuscularly into the upper outer quadrant of the buttock. Rubbing the buttocks after the injection is not recommended.

Ampoules bicillin 5

Intramuscularly.

Adults for 1.2 million AD + 300 thousand AD once every 4 weeks.

Pre-school children - 480 thousand AD + 120 thousand AD once every 3 weeks, children older than 8 years - 960 thousand AD + 240 thousand AD once every 4 weeks.

Rules for the preparation, dilution and administration of a solution Bicillin 5

To prepare the suspension use sterile water for injection, isotonic sodium chloride solution or 0.25-0.5% solution of procaine (novocaine).

Suspension of Bicillin 5 is prepared aseptically, immediately before use (ex tempore): 5-6 ml of solvent are slowly introduced into the vial with the drug under pressure slowly (at a rate of 5 ml in 20-25 seconds). The contents of the vial are mixed and shaken along the longitudinal axis of the vial to form a homogeneous suspension. Bubbles are allowed on the surface of the suspension near the walls of the vial. Suspension of Bicillin 5 immediately after preparation is injected deeply intramuscularly into the upper outer quadrant of the gluteus muscle. Rubbing the gluteus muscle after injection is not recommended. When the introduction is delayed, the physical and colloidal properties of the suspension change immediately after preparation,as a result of which its movement through the syringe needle can be impeded.

It is recommended to stop breastfeeding when it is necessary to prescribe the drug.

Use in children

It is possible to use an antibiotic in children according to the indications in age-related dosages.

special instructions

When developing allergic reactions should immediately stop treatment.

With the development of anaphylactic shock, anti-shock therapy (norepinephrine, glucocorticosteroids (GCS), ventilator) is indicated.

With caution apply in patients with impaired renal function, pseudomembranous colitis, in case of predisposition to allergic reactions (especially with drug allergies), with increased sensitivity to cephalosporins (due to the possibility of developing a cross-allergy).

It is not recommended to use for the treatment of neurosyphilis (since the necessary concentration in the blood plasma is not achieved). Intravenous and endolumbral administration is not allowed.

With a random intravascular injection, there may be a transient sense of depression, anxiety, and visual impairment (Ouenier syndrome). In order to avoid accidental intravascular administration of the drug, it is recommended to perform aspiration before intramuscular injection in order to detect a possible needle drop into the vessel.

In connection with the possible development of fungal lesions, it is advisable to prescribe antifungal drugs (nystatin, levorin), as well as Ascorbic acid and B vitamins.

It should be borne in mind that the use of the drug in insufficient doses or early cessation of treatment leads to the emergence of resistant strains of pathogens.

Drug Interactions

When used simultaneously with NSAIDs (in particular, with indomethacin, phenylbutazone and salicylates), one should keep in mind the possibility of developing a competitive inhibition of excretion of drugs from the body.

When used simultaneously with bactericidal antibiotics (including cephalosporins, cycloserine, vancomycin, rifampicin, aminoglycosides), synergy is noted; with bacteriostatic antibiotics (including macrolides, chloramphenicol, lincosamides, tetracyclines) - antagonism.

Bicillin reduces the effectiveness of oral contraceptives and ethinyl estradiol (risk of development of breakthrough bleeding).

With the simultaneous use of Bicillin and allopurinol, the risk of developing allergic reactions increases.

Pharmacokinetic interaction

With the simultaneous use of diuretics, allopurinol, phenylbutazone, NSAIDs reduce tubular secretion and increase the concentration of penicillin.
