In this article, you can read the instructions for using the drug Vancomycin. There are reviews of visitors to the site - consumers of this medication, as well as opinions of doctors of specialists on the use of Vancomycin antibiotic in their practice. A big request is to actively add their feedback on the drug: the medicine helped or did not help get rid of the disease, which were observed complications and side effects, possibly not declared by the manufacturer in the annotation. Analogues of Vancomycin in the presence of existing structural analogues.Use for the treatment of bacterial infections in adults, children, as well as in pregnancy and lactation. Composition of the preparation.

Vancomycin - antibiotic group of glycopeptides. Has a bactericidal effect. Violates the synthesis of the cell wall, the permeability of the cytoplasmic membrane and the synthesis of RNA bacteria. It is active against gram-positive bacteria: Staphylococcus spp. (Staphylococcus aureus) (including strains producing penicillinase and methicillin-resistant strains), Streptococcus spp. (streptococcus), Enterococcus spp. (Enterococcus), Corynebacterium spp., Listeria spp. (Listeria), Actinomyces spp., Clostridium spp. (including Clostridium difficile).

There was no cross-resistance with antibiotics of other groups.

Composition

Vancomycin (in the form of hydrochloride) + auxiliary substances.

Pharmacokinetics

Widely distributed in most tissues and body fluids. Poor penetration of the blood-brain barrier (BBB), but with inflammation of the meninges, permeability increases. Penetrates through the placental barrier. Binding to plasma proteins is 55%. 80-90% is excreted in the urine, a small amount is excreted with bile.

Indications

Infectious and inflammatory diseases of severe course,caused by susceptible to vancomycin pathogens (with intolerance or inefficiency of therapy with other antibiotics, including penicillins or cephalosporins):

  • sepsis;
  • endocarditis (as a monotherapy or as part of combined antibiotic therapy);
  • pneumonia;
  • abscess of the lungs;
  • meningitis;
  • infection of bones and joints;
  • infections of the skin and soft tissues;
  • pseudomembranous colitis caused by Clostridium difficile.

Forms of release

Powder for solution for infusion (injections in ampoules for injection) 500 mg and 1 g.

Lyophilizate for the preparation of a solution for infusions of 500 mg and 1 g.

There are no other dosage forms, be it pills or suspensions.

Instructions for use and dosage

Enter intravenously drip (in the form of a dropper). Adults-500 mg every 6 hours or 1 g every 12 hours. To avoid collapoid reactions, the duration of the infusion should be at least 60 minutes. Children - 40 mg / kg per day, each dose should be administered at least 60 minutes. In patients with impaired renal excretory function, the dose is reduced with allowance for the CK values.

Depending on the etiology of the disease, vancomycin can be taken orally.For adults, the daily dose is 0.5-2 g in 3-4 doses, for children - 40 mg / kg in 3-4 doses.

The maximum daily intake for adults with intravenous administration is 3-4 g.

Side effect

  • heart failure;
  • tides;
  • a decrease in blood pressure;
  • shock (these symptoms are mainly associated with rapid infusion of drug substance);
  • neutropenia, eosinophilia, thrombocytopenia, agranulocytosis;
  • nausea;
  • interstitial nephritis;
  • change in functional renal tests;
  • impaired renal function;
  • vertigo;
  • tinnitus;
  • ototoxic effects;
  • exfoliative dermatitis;
  • benign bubble dermatosis;
  • itching dermatosis;
  • rash;
  • hives;
  • Stevens-Johnson syndrome;
  • toxic epidermal necrolysis;
  • vasculitis;
  • anaphylactoid reactions;
  • chills;
  • drug fever;
  • necrosis of tissues at injection sites;
  • pain in the injection site;
  • thrombophlebitis;
  • the development of "red neck syndrome" associated with the release of histamine: erythema, skin rash, redness of the face, neck, upper trunk, hands, heart palpitations, nausea, vomiting, chills, fever, fainting.

Contraindications

  • neuritis of the auditory nerve;
  • severe renal dysfunction;
  • 1 trimester of pregnancy;
  • lactation period (breastfeeding);
  • increased sensitivity to vancomycin.

Application in pregnancy and lactation

The use in the first trimester of pregnancy is contraindicated because of the risk of developing nephro- and ototoxicity. The use of vancomycin in the 2nd and 3rd trimesters is possible only for vital indications.

If it is necessary to use lactation, breastfeeding should be discontinued.

Use in children

When used in newborns, it is necessary to monitor the concentration of vancomycin in the blood plasma.

Application in elderly patients

When used in elderly patients, it is necessary to monitor the concentration of vancomycin in the blood plasma.

special instructions

Use with caution for mild to moderate renal impairment, hearing impairment (including anamnesis), in the 2nd and 3rd trimesters of pregnancy. During the treatment period, patients with kidney disease and / or damage to 8 pairs of CMN should be monitored for kidney function, hearing.

Intramuscular administration of vancomycin is not allowed because of the high risk of tissue necrosis.

When used in newborns or elderly patients, it is necessary to monitor the concentration of vancomycin in the blood plasma.

Drug Interactions

The simultaneous use of vancomycin and local anesthetics can lead to the development of erythema, histamine-like hot flashes and anaphylactic shock.

With the simultaneous use of vancomycin with aminoglycosides, amphotericin B, cisplatinum, cyclosporin, furosemide, polymyxins, an increase in oto and nephrotoxic effect is noted.
