Composition of the preparation.

Neufuan - combined preparation for topical application. Lidocaine hydrochloride has a local anesthetic effect. Neomycin sulfate is an antibiotic from the group of aminoglycosides with a broad spectrum of bactericidal action. It is active against gram-positive cocci, gram-negative rods and actinomycetes. Fluocinolone acetonide - synthetic glucocorticosteroid (GCS), with local application has anti-inflammatory effect. Does not affect the antibacterial activity of neomycin and enhances the local anesthetic effect of lidocaine.

Composition

Neomycin sulfate + fluocinolone acetonide + lidocaine hydrochloride + excipients.

Pharmacokinetics

Lidocaine hydrochloride is absorbed by the mucous membranes and enters the systemic circulation, after which it is very rapidly metabolized in the liver. Less than 10% of lidocaine is excreted unchanged in urine.

Neomycin is practically not absorbed with topical application, in any case in the amount that can lead to the development of systemic side effects.

Fluorocinolone acetonide is not absorbed and does not cause systemic effects, except for application to damaged surfaces.

Indications

  • diagnostic and therapeutic procedures (including cystoscopy, bladder catheterization, urethral expansion, colposcopy, rectoscopy, colonoscopy);
  • acute and chronic nonspecific inflammatory diseases of the lower urinary tract (including acute cystitis, exacerbation of chronic cystitis, urethritis);
  • Infectious and inflammatory diseases of the rectum (including hemorrhoids, acute anal fissure, cryptite, anusitis);
  • inflammatory skin changes with active fistulas of the rectum.

Forms of release

Gel for topical application (sometimes mistakenly called ointment or cream) 5 mg + 250 μg + 25 mg.

Instructions for use and dosing regimen

When carrying out endourethral procedures, the gel is squeezed out of the tube directly into the urethra by squeezing the tube and simultaneously massaging the posterior wall of the urethra. Then a clamp is applied to the urethra, which is removed after 10 minutes, after which endourethral procedures are performed.

To simplify the administration of the drug and relieve the pain that can occur when the metal part of the tube tip touches the pain zones, a plastic extension is attached to the tube, screwed onto its tip immediately before use.

It is recommended to inject the gel a few minutes before the start of diagnostic or therapeutic manipulation, and also immediately after their termination.

The contents of one tube are enough to fill the entire urethra, however in some cases the dose can be changed depending on the extent of the lesion and the desired effect.

When carrying out endoscopic research in proctology (sigmoidoscopy, colonoscopy), the gel is injected into the anal canal 10-15 minutes before the procedure and again immediately before its beginning.

With infectious and inflammatory diseases of the lower urinary tract, the gel is injected into the urethra 1-2 times a day for 5-10 days.

With infectious and inflammatory diseases of the rectum, the gel is applied to the affected surface of the skin and mucous membrane and injected into the anal canal with the help of an extension (after evacuation of the intestine and hygienic procedures) 3 times a day for 10-14 days.

Side effect

  • itching;
  • burning sensation;
  • Hyperemia (redness) of the skin.

Contraindications

  • increased sensitivity to lidocaine and other anesthetics of the amide series;
  • hypersensitivity to neomycin, fluocinolone and other components of the drug.

Caution should be used in children, elderly patients and weakened patients.

Application in pregnancy and lactation

Adequate and strictly controlled studies on the use of Nefuana during pregnancy and lactation were not conducted. Therefore, the appointment of the drug during pregnancy (especially in the early stages) and during lactation (breastfeeding) is undesirable or should be carefully justified.

Use in children

Neutral for the treatment of children is used with caution.

special instructions

Efficacy and safety Nefuana, like other local anesthetic, depends on the correct dosage and techniques of its use.

To prevent side effects should be, if possible, use the minimum effective dose of the drug.

For a complete anesthesia of the urethra, in carrying out any instrumental interventions, usually 1 tube of Nefuana is sufficient.

The risk of side effects increases with very long or continuous use, use in high doses or in the presence of extensive lesions of the skin and mucous membranes.
