Zipromed - an antibacterial drug from the group of fluoroquinolones for topical application in ophthalmology and otorhinolaryngology. Has a broad spectrum of antibacterial action, has a bactericidal effect. The mechanism of action is associated with the ability to inhibit the DNA enzyme of the bacterial DNA, which leads to a disruption in the replication of DNA and the synthesis of cellular bacterial proteins. Ciprofloxacin (the active ingredient of the drug Tsipromed) acts both on multiplying microorganisms, and on those in rest.

 

It is active against gram-negative microorganisms: Escherichia coli (E. coli), Salmonella spp. (salmonella), Shigella spp. (Shigella), Proteus spp. (indole-positive and indole-negative strains), Morganella morganii, Citrobacter spp., Klebsiella spp., Enterobacter spp., Vibrio spp., Campylobacter spp., Hafnia spp., Providencia stuartii, Haemophilus influenzae, Pasteurella multocida, Pseudomonas spp., Gardnerella spp., Legionella pneumophila, Neisseria spp., Moraxella catarrhalis, Acinetobacter spp., Brucella spp., Chlamydia spp. (chlamydia).

 

Gram-positive microorganisms are also sensitive to ciprofloxacin: Staphylococcus spp. (Staphylococcus aureus), Streptococcus pyogenes (Streptococcus), Streptococcus agalactiae, Corynebacterium diphtheriae, Listeria monocitogenes.

Pharmacokinetics

 

With local application of the drug, it penetrates well into the tissues of the eye. After 2 hours the concentration of the drug begins to decrease, while its antibacterial action in the tissues of the cornea persists up to 6 hours, in the moisture of the anterior chamber - up to 4 hours. After instillation, systemic absorption of the drug is possible. The drug is excreted by the kidneys in unchanged form - up to 50%, and in the form of metabolites - up to 10%; through the intestine - about 15%. Some part of the drug is excreted in breast milk.

 

Indications

 

Infectious and inflammatory diseases of the eye and its appendages caused by microorganisms sensitive to the preparation:

  • acute and subacute conjunctivitis;
  • keratitis;
  • anterior uveitis;
  • blepharitis and other inflammatory diseases of the eyelids;
  • dacryocystitis;
  • prevention and treatment of infectious complications in traumas of the eye and its appendages and in operations on the eyeball.

 

Treatment of acute and chronic infectious diseases of the ear:

  • otitis externa;
  • acute and chronic otitis media;
  • prevention of infectious otitis before and after surgery, with ear trauma, removal of a foreign body from the external auditory canal, accompanied by damage to the ear tissues.

 

Forms of release

 

Eye drops 0.3%.

 

Drops of ear 0.3%.

 

Forms in the form of tablets or powder do not exist.

 

Instructions for use and how to use them

 

Eye Drops

 

The drug is prescribed 1-2 drops in the conjunctival sac of the affected eye. The frequency of instillations depends on the severity of the inflammatory process.

 

With acute bacterial conjunctivitis, simple, scaly and ulcerative blepharitis, Cipromed is prescribed 4-8 times a day, depending on the severity of the inflammatory process. The course of treatment is from 5 to 14 days.

 

In keratitis, the drug is prescribed 1 drop at least 6 times a day. If there is a positive effect, the duration of therapy depends on the severity of the corneal lesion and is usually 2-4 weeks.

 

If a corneal injury caused by Pseudomonas aeruginosa is affected, tsipromed is prescribed 1 drop at least 8-12 times a day. The duration of therapy depends on the course of the disease and is usually about 2-3 weeks.

 

With anterior uveitis, the drug is prescribed 1 drop 8-12 times a day.

 

With acute dacryocystitis and canaliculitis Tsipromed appoint 1 drop 6-12 times a day, with chronic - 4-8 times a day.

 

For the prevention of secondary infection in cases of eye injuries and its appendages, tsipromed is prescribed 1 drop 4-8 times a day for 1-2 weeks.

 

For prevention of inflammatory diseases after surgery with perforation of the eyeball, tsipromed is prescribed 1 drop 4-6 times a day during the entire postoperative period (usually from 5 days to 1 month).

 

Ear drops

 

Before applying the drug, the external auditory canal should be sanitized (rinse and drain the ear canal). The drug is instilled in 5 drops into the external auditory meatus 3 times a day. Drops should be heated to body temperature.

 

After the disappearance of the symptoms of the disease should continue treatment for 48 hours.

Contraindications

  • pregnancy;
  • lactation (breastfeeding);
  • children's age up to 1 year (eye drops), up to 15 years (ear drops);
  • hypersensitivity to the components of the drug or to other fluoroquinolones.

 

Application in pregnancy and lactation

 

Tsipromed is contraindicated for use in pregnancy. If you need to use the drug during lactation, breastfeeding should be discontinued.

 

Use in children

 

Contraindicated in children under the age of 1 year (eye drops) and at the age of 15 years (ear drops).

When wearing soft contact lenses, do not use eye drops Tsipromed, t. the preservative included in their composition can be deposited in soft contact lenses and have an adverse effect on the eye tissue. Rigid contact lenses should be removed before instillation of the drug and set them again after 15 minutes.
