Analogues of Hemomycin in the presence of existing structural analogues.Use for the treatment of angina, pneumonia and other infectious diseases in adults, children, as well as during pregnancy and lactation. Composition and interaction of the drug with alcohol.
Hemomycin - broad spectrum antibiotic. Azithromycin (the active substance of the drug Chemomycin) is a representative of a subgroup of macrolide antibiotics - azalides. In high concentrations has a bactericidal effect.
Hemomycin is active against aerobic Gram-positive and Gram-negative bacteria, as well as anaerobic bacteria.
The drug is active against intracellular microorganisms: Chlamydia trachomatis, Mycoplasma pneumoniae, Ureaplasma urealyticum, Borrelia burgdorferi, and also with respect to Treponema pallidum.
Gram-positive bacteria resistant to Erythromycin are resistant to the drug.
Composition
Azithromycin (in the form of dihydrate) + auxiliary substances.
Pharmacokinetics
Chemomycin is rapidly absorbed from the digestive tract, which is due to its resistance in acidic environment and lipophilicity. Bioavailability is 37%. Azithromycin well penetrates the respiratory tract, organs and tissues of the urogenital tract, into the prostate gland, the skin and soft tissues. The ability of azithromycin to accumulate mainly in lysosomes is especially important for the elimination of intracellular pathogens. It is proved that phagocytes deliver azithromycin to the site of infection, where it is released during phagocytosis. The concentration of azithromycin in the foci of infection is significantly higher than in healthy tissues (an average of 24-34%) and correlates with the degree of inflammatory edema. Despite the high concentration in phagocytes, azithromycin does not significantly affect their function. Azithromycin is retained in bactericidal concentrations in the inflammatory focus within 5-7 days after administration of the last dose, which allowed the development of short (3-day and 5-day) treatment courses.

Rules for the preparation of suspension
In the vial containing the powder, gradually add water (distilled or boiled and chilled) to the mark. The contents of the vial are shaken carefully until a uniform suspension is obtained. If the level of the prepared suspension is below the label on the vial label, re-add water to the label and shake.
The prepared suspension is stable at room temperature for 5 days.
Before use, the suspension should be shaken.
Immediately after taking the suspension, the child should be allowed to drink several sips of liquid (water, tea) in order to wash off and swallow the remaining suspension in the oral cavity.
Ampoules
The drug should be used only in inpatient medical institutions. Recommended doses for intramural administration of chemomycin in the treatment of adults and patients over 16 years of age with the following diseases:
Community-acquired pneumonia (EP)
500 mg once a day intramuscularly for at least 2 days. In / in the administration should be replaced by the subsequent oral administration of azithromycin in the form of a single daily dose of 500 mg until the complete completion of a 7-10 day general course of treatment.
Infectious-inflammatory diseases of the pelvic organs
500 mg once a day intravenously for at least 2 days. In / in the administration should be replaced by the subsequent oral administration of azithromycin in the form of a single daily dose of 250 mg until the complete completion of a 7-day general course of treatment.
The timing of the transition to oral treatment is determined in accordance with the data of the clinical examination.
For patients with moderate impairment of liver and kidney function (creatinine clearance> 40 ml / min) dose adjustment is not needed.
Rules for the preparation of solution
Solution for infusion is prepared in 2 stages:
Stage 1 - preparation of the reconstituted solution: 4.8 ml of sterile water for injections is added to a bottle with 500 mg of the preparation and thoroughly shaken until the powder is completely dissolved. In 1 ml of the resulting solution contains 100 mg of azithromycin. The prepared solution remains stable for 24 hours at room temperature.
Stage 2 - dilution of the reconstituted solution (100 mg / ml) is carried out immediately before administration.
The reconstituted solution is introduced into a vial with a solvent (0.9% sodium chloride solution, 5% dextrose, Ringer's solution) until a final concentration of azithromycin is 1-2 mg / ml in the infusion solution.
A solution of hemomycin can not be administered intravenously either by jet or by intramuscular injection. It is recommended to enter the prepared solution IV in the form of infusion, drip (dropper) (at least 1 hour).
Before injection, the solution is visually inspected. If the finished solution contains particles of the substance, then it should not be used. The prepared solution is stable at room temperature for 24 hours.

special instructions
Do not take the drug while eating.
It is recommended that the interval is observed for at least 2 h between taking Chemomycin and antacid preparations.
After the abolition of treatment, hypersensitivity reactions may persist in some patients, which requires specific therapy and medical control.
Drug Interactions
With the simultaneous use of chemomycin and antacids (aluminum and magnesium-containing), the absorption of azithromycin is slowed.
Ethanol (alcohol) and food slow down and reduce the absorption of azithromycin.
With the co-administration of Warfarin and azithromycin (in normal doses), no changes in prothrombin time have been detected, but considering that macrolides and warfarin interact may enhance the anticoagulant effect, patients need careful monitoring of prothrombin time.
The combined use of Hemomycin and Digoxin increases the concentration of the latter.
With the simultaneous use of azithromycin with ergotamine and dihydroergotamine, there is an increase in the toxic effect of the latter (vasospasm, dysesthesia).
Co-administration of triazolam and azithromycin decreases clearance and enhances the pharmacological action of triazolam.
Hemomitsin slows down and increases the plasma concentration and toxicity of cycloserine, anticoagulants, methylprednisolone, felodipine, as well as medicaments suffer from microsomal oxidation (carbamazepine, terfenadine, cyclosporine, hexobarbital, ergot alkaloids, valproate, disopyramide, bromocriptine, phenytoin, oral hypoglycemic agents, theophylline and other xanthine derivatives) due to the azithromycin inhibition of microsomal oxidation in hepatocytes.
Lincosamines weaken the effectiveness of azithromycin, and Tetracycline and chloramphenicol - strengthen.
Pharmaceutical interaction
Pharmaceutically, Hemomycin is incompatible with heparin.
