Baneocin - Combined antibacterial drug for external use. Contains two antibiotics, which have a bactericidal effect, neomycin and bacitracin.
Bacitracin is a polypeptide antibiotic that inhibits the synthesis of the bacterial cell membrane.
Neomycin is an antibiotic, an aminoglycoside, which inhibits the synthesis of bacterial proteins.
Bacitracin is active against gram-positive and some gram-negative microorganisms. Resistance to bacitracin is rare. Has good tissue tolerance; Inactivation by biological products, blood and tissue components is not noted.
Neomycin is active against gram-positive and gram-negative bacteria.
By using a combination of these two antibiotics, a broad spectrum of action of the drug is achieved and synergism of action against a number of microorganisms, for example, staphylococci.

Pharmacokinetics
The active substances, as a rule, are not absorbed (even damaged by the skin), nevertheless, their skin contains high concentrations.When applying the drug to extensive areas of skin lesions, the possibility of systemic absorption of the drug should be taken into account.
Indications
Treatment of infectious and inflammatory skin diseases caused by microorganisms sensitive to the preparation:
Powder
- bacterial skin infections of limited prevalence, incl. wetting contagious impetigo, infected trophic ulcers of the lower extremities, infected eczema, bacterial diaper dermatitis, secondary bacterial infection in diseases caused by Herpes simplex, Varicella zoster (including chicken pox);
- prevention of umbilical infection in newborns;
- prevention of infection after surgical (including dermatological) procedures: in the postoperative period (after excision of tissues, cauterization, episiotomy, treatment of cracks, wiping wounds and sutures).
Ointment
- focal skin infections, including furuncles, carbuncles (after surgical treatment), staphylococcal sycosis, deep folliculitis, purulent hydradenitis, paronychia;
- bacterial skin infections of limited prevalence, incl. contagious impetigo, infected ulcers of the lower limbs, secondarily infected eczema,Secondary infection with dermatoses, cuts, abrasions, burns, in cosmetic surgery and skin transplantation (also for the purpose of prevention and for impregnation of dressings);
- prevention of infection after surgical interventions (as part of combination therapy in the postoperative period).
Forms of release
Ointment for external use (sometimes mistakenly called cream).

Instructions for use and how to use them
The drug is applied a thin layer on the affected areas: powder - 2-4 times a day, ointment - 2-3 times a day (to enhance the effectiveness of applying ointment under the bandage).
The use of ointment with turundas is preferred for the local treatment of infected cavities and wounds (including bacterial infections of the external auditory canal without perforation of the tympanic membrane, wounds or surgical incisions healing with secondary tension).
For burns of more than 20% of the body surface, the powder should not be used more than once a day, especially if the kidney function decreases (because it is possible to absorb the active ingredient).
With external application, the dose of neomycin should not exceed 1 g per day (corresponding to 200 g of powder or ointment) for 7 days. At the repeated course the maximum dose is not more than 100 g.
Side effect

- development of oto- and nephrotoxic effects and disorders of neuromuscular conduction in systemic action.
Contraindications
- extensive skin lesions (risk of ototoxic effect, accompanied by hearing loss);
- pronounced impairment of the excretory function of the kidneys (due to cardiac or renal insufficiency) with the existing diseases of the cochlear-vestibular apparatus (if systemic absorption of the drug is possible);
- eye diseases (for powder);
- hypersensitivity to bacitracin, neomycin or other aminoglycosides.
Application in pregnancy and lactation
The use of Baneocin during pregnancy and lactation is possible only if the intended benefit to the mother exceeds the potential risk to the fetus or baby.

Avoid contact with the eyes.
When used in doses that are significantly higher than recommended, due to possible absorption, attention should be paid to symptoms indicating nephro- or ototoxic reactions.
Because the risk of toxic effects increases with decreasing liver and / or kidney function, blood and urine tests should be performed with patients with hepatic and / or renal insufficiency together with an audiometric examination before and during therapy with Baneocin.
With the possible absorption (extensive violations of the integrity of the skin), it is necessary to monitor the possible appearance of signs of neuromuscular blockade, especially in patients with acidosis, severe myasthenia gravis or other neuromuscular diseases.
With the development of neuromuscular blockade, calcium preparations or neostigmine are indicated.
With prolonged use of the drug, it is necessary to monitor the possible excessive growth of resistant microorganisms. If necessary, appropriate treatment should be prescribed.
If the drug is used in children, patients with impaired liver and kidney function, and also with a large area of the treated surface, prolonged use and deep skin lesions, it is necessary to consult a doctor in advance.
With the development of allergic reactions and superinfection, the drug should be canceled.
Drug Interactions
If there is a systemic absorption, then with the simultaneous appointment of cephalosporins or antibiotics - aminoglycosides, the risk of developing nephrotoxic reactions increases.
With the simultaneous use of Baneocin with ethacrynic acid or furosemide, the risk of oto and nephrotoxic reactions increases.
In the case of development of systemic absorption with the simultaneous use of Baneocin with opioid analgesics, anesthetics and muscle relaxants, the risk of neuromuscular blockade increases.
There were no incidents of incompatibility between bacitracin and neomycin.
