Sofradeks - instructions for use, reviews, analogs and forms of release (eye and ear drops 5 ml) drugs for the treatment of blepharitis, conjunctivitis and otitis in adults, children and pregnancy. Composition

In this article, you can read the instructions for using the drug Sofradex. There are reviews of visitors to the site - consumers of this medication, as well as opinions of physicians specialists on the use of Софрадекса in their practice. A big request is to actively add their feedback on the drug: the medicine helped or did not help get rid of the disease, which were observed complications and side effects, possibly not declared by the manufacturer in the annotation. Analogues of Софрадекса at presence of available structural analogues. Use for the treatment of blepharitis, conjunctivitis and otitis in adults, children, as well as during pregnancy and lactation.Composition of the preparation.

Sofradex - has antibacterial bactericidal action of a wide spectrum, is active against various gram-positive and gram-negative microorganisms (due to Framicetin and gramicidin). Has anti-inflammatory, anti-allergic and antipruritic properties (glucocorticoid dexamethasone). When instilled in the eye reduces pain, burning, photophobia and lacrimation, characteristic of the corresponding pathological processes. The effect is due to the constituent chemicals.

Framicetin sulfate is an antibiotic from the aminoglycoside group, it acts bactericidal. It has a wide spectrum of antibacterial action, is active against gram-positive microorganisms, including Staphylococcus aureus, and most clinically significant Gram-negative microorganisms (E. coli, dysentery rod, proteus, etc.). It is not effective for streptococci. Does not affect pathogenic fungi, viruses, anaerobic flora. Stability of microorganisms to framicetin sulfate develops slowly. Gramicidin - has a bactericidal and bacteriostatic effect,expands the spectrum of the antimicrobial action of Framicetin due to its activity against staphylococci, since it also has an anti-staphylococcal effect.

Dexamethasone is a glucocorticosteroid (GCS), which has a pronounced anti-inflammatory, antiallergic and desensitizing effect. Dexamethasone suppresses inflammatory processes, inhibiting the release of inflammatory mediators, the migration of mast cells and reducing the permeability of capillaries. When instilled in the eyes reduce pain, burning, lacrimation, photophobia. When instilled in the ears reduces the symptoms of otitis externa ear (reddening of the skin, pain, itching, burning in the external ear canal, a feeling of stuffiness of the ear).

Composition

Framicetin sulfate + Gramicidin + Dexamethasone (in the form of sodium metasulfobenzoate) + auxiliary substances.

Pharmacokinetics

With topical application, systemic absorption is low.

Framicetin sulfate can be absorbed through inflamed skin or open wounds. When entering the systemic bloodstream, it is quickly excreted by the kidneys unchanged.

When ingesting dexamethasone can be absorbed quickly from the digestive tract.

Indications

Bacterial diseases of the eye:

  • blepharitis;
  • conjunctivitis;
  • keratitis (without epithelial damage);
  • iridocyclitis;
  • sclerites, episclerites;
  • infected eczema of the skin of the eyelids.

Bacterial diseases of the ears:

  • Otitis of the external ear.

Forms of release

Eye and ear drops 5 ml.

Other forms of the drug, such as ointment or tablets do not exist, the preparation is unique in the composition of the incoming components.

Instructions for use and how to use them

In case of eye diseases: for a mild infection, 1-2 drops of the drug are injected into the conjunctival sac of the eye every 4 hours. In case of a serious infectious process, the drug is instilled every hour. As the inflammatory phenomena decrease, the frequency of instillations of the drug decreases.

In case of ear diseases: 2-3 drops are dripped 3-4 times a day, a gauze pad moistened with solution can be placed in the external auditory canal.

The drug intranasal (in the nose) is not used.

Side effect

  • allergic reactions;
  • irritation;
  • burning;
  • pain;
  • itching;
  • dermatitis;
  • increased intraocular pressure with the development of the glaucoma symptom complex (optic nerve damage, visual acuity and appearance of visual field defects),so with more than 7-day use of drugs containing glucocorticosteroids, the intraocular pressure should be measured regularly;
  • development of posterior supposapsular cataract (especially with frequent instillation);
  • thinning of the cornea or sclera, which can lead to perforation;
  • attachment of secondary (fungal) infection.

Contraindications

  • increased individual sensitivity to any of the components of the drug;
  • viral or fungal infections, tuberculosis, purulent inflammation of the eyes, trachoma;
  • violation of the integrity of the epithelium of the cornea and thinning of the sclera;
  • herpetic keratitis (dendritic corneal ulcer) (possibly an increase in the size of the ulcer and a significant deterioration in vision);
  • glaucoma;
  • perforation of the tympanic membrane (penetration of the drug into the middle ear can lead to the development of ototoxic action);
  • pregnancy and the period of breastfeeding;
  • infants.

Application in pregnancy and lactation

The drug is contraindicated during pregnancy and lactation.

Use in children

The drug is contraindicated in infants.

Use with caution in young children.

special instructions

With long-term use of the drug, as with the long-term use of other antimicrobial agents, it is possible to develop superinfections caused by microorganisms resistant to the preparation, including fungi.

Continuous instillation of the drug in the eyes can lead to thinning of the cornea with the development of its perforation, as well as an increase in intraocular pressure. Treatment with drugs containing corticosteroids should not be repeated or prolonged without regular monitoring of intraocular pressure, eye examination for cataracts or secondary infections.

Local glucocorticosteroids (SCS) should never be used in patients with eye hyperemia of unidentified etiology, Inadequate use of the drug may lead to significant visual impairment.

Framicetin sulfate, which is part of the drug, is an antibiotic from the aminoglycoside group, which is characterized by the development of nephro- and ototoxic effects in systemic application or local application to an open wound or damaged skin. These effects depend on the dose and are increased and renal or hepatic insufficiency.Although the development of these effects has not been observed with instillation of the drug in the eye, it should be taken into account the possibility of their occurrence in the case of topical application of high doses of the drug in children.

The duration of the drug should not exceed 7 days, except in cases of obvious positive dynamics of the disease, because long-term use of SCS, which is part of it, can mask hidden infections, and long-term use of antimicrobial components - contribute to the development of a stable flora.

The bottle must be closed after each use. Do not touch the tip of the pipette to the eye. After opening the vial, the drug should be used within 1 month.

Impact on the ability to drive vehicles and manage mechanisms

Patients who, after instillation in the eye of the drug, temporarily lose their clear vision, are not recommended immediately after instillation of the drug to drive a car or work with complex equipment, machines or some other complex equipment that requires a clear vision.

Drug Interactions

Do not use framicetin sulfate along with other antibiotics that have ototoxic and nephrotoxic effects (streptomycin, monomycin, kanamycin, gentamicin).

I used this drug in Thailand.
There was a terrible otitis of the right ear, I had to call for insurance.
The insurance company registered my insured event, told me which hospital to go for help and sent a letter of guarantee to this hospital in my case.
In the hospital, the ear was washed on some kind of apparatus, given me a bunch of pills and cotton swabs. Including Sofradex. Dripped it for about a week, along with eating the prescribed tablets. The effect occurred somewhere for 2-3 days. The effect is satisfied. I recommend.
