Pharmacokinetics

 

The pharmacokinetics of diclofenac and misoprostol in combination are similar to the pharmacokinetics of these products when administered as separate tablets. Misoprostol is rapidly absorbed and metabolized to the active metabolite - misoprostol acid, the latter has a decay period of about 30 minutes.70% of the administered dose of misoprostol is excreted in the urine as inactive metabolites. Diclofenac sodium is also rapidly and almost completely absorbed, reversibly binds to plasma proteins; from 40 to 60% is excreted in the urine, the rest - with bile and feces. Pharmacokinetic interaction between these two substances is not observed. After repeated administration of Arthrochet, no cumulation of diclofenac, misoprostol or misoprostol acid occurs.

Application in pregnancy and lactation

 

Contraindicated in the use of Artrothe during pregnancy and lactation (breastfeeding). Women of childbearing age should not be treated with Artrothek until pregnancy is excluded; they also need to use reliable contraceptive measures throughout the therapy period. If pregnancy is expected, then this drug should be canceled.

 

Use in children

Application in elderly patients

 

Use with caution in elderly patients. Drug Interactions

 

Diclofenac is displaced by Acetylsalicylic acid from the sites of its binding to albumins, which leads to a decrease in the concentration of diclofenac in plasma, and also to a decrease in its maximum concentration in plasma. In this regard, the simultaneous use of arthrochet and acetylsalicylic acid is not recommended.

 

In patients who received both Digoxin and diclofenac, an increased level of digoxin was recorded.Therefore, patients receiving both digoxin and Arthrotek should be under medical supervision to detect the toxic effect of digoxin.

 

Since simultaneous use of Arthrochet with potassium-sparing diuretics may increase potassium levels in serum, in such cases it is necessary to monitor the level of this indicator.

 

With the simultaneous administration of an arthrothe with angiotensin-converting enzyme (ACE) inhibitors, renal function may worsen.

 

Arthrochet should be administered with caution to patients receiving insulin.

Diclofenac reduces renal clearance of lithium and increases the concentration of lithium in plasma. Therefore, patients receiving lithium, Arthrochet should be administered with caution.
