Allomaron - a drug that affects the exchange of uric acid. Allomaron contains two active ingredients: Allopurinol and benzbromarone.

 

Allopurinol disrupts the synthesis of uric acid. It is a structural analogue of hypoxanthine. It inhibits the enzyme xanthine oxidase, which is involved in the conversion of hypoxanthine to xanthine and xanthine into uric acid. This is due to a decrease in the concentration of uric acid and its salts in the liquid media of the body and urine, which helps dissolve the existing urate deposits and prevents their formation in tissues and kidneys. When taking allopurinol, the excretion of urine hypoxanthine and xanthine increases.

 

Benzbromarone has a hypouricemic and uricosuric effect. Increases the excretion of uric acid by the kidneys, suppressing the reabsorption of uric acid in the renal tubules. To some extent suppresses the synthesis of purines.

 

Allomaron reduces the elevated level of uric acid in the blood plasma, eliminates and prevents its deposition, prevents kidney damage and gout attacks.

Pharmacokinetics

 

After taking the tablets, allopurinol is almost completely absorbed (90%) from the gastrointestinal tract (GIT). Metabolized with the formation of alloxanthin, which retains the ability to inhibit xanthine oxidase long enough. About 20% of the accepted dose of allopurinol is excreted through the intestine, the rest - by the kidneys. After ingestion, the absorption of benzbromarone from the gastrointestinal tract is incomplete. It is highly associated with plasma proteins. Metabolised in the liver and excreted mainly with feces, in small amounts determined in urine.

 

Indications

  • Hyperuricemia (increased uric acid in the blood plasma) of different genesis;
  • gout, including gouty arthritis.

Tablets coated with 100 mg + 20 mg.

 

Instructions for use and dosing regimen

 

Allomaron taken inside, 1-3 tablets a day after meals, with the mandatory intake of at least 1.5-2 liters of fluid per day (under the control of diuresis). Treatment should begin with 1 tablet a day with a gradual increase in dose.

 

Side effect

  • gastrointestinal disorders, diarrhea;
  • vasculitis (inflammation of blood vessels) with skin manifestations and fever;

Contraindications

  • hypersensitivity to the components of the drug;

  • severe renal dysfunction (with creatinine level in the serum above 132.6 μmol / l (1.5 mg / 100 ml) or with a glomerular filtration rate below 40 ml per minute);
  • pregnancy;
  • breast-feeding;
  • age younger than 14 years.

 

Application in pregnancy and lactation

 

Allomaron is contraindicated in pregnancy and lactation.

 

Use in children

 

Contraindicated in children under 14 years.

 

special instructions

 

In severe renal impairment (serum creatinine level more than 1.5 mg% and glomerular filtration rate below 40 ml per minute) is not used. In these cases, allopurinol is recommended without benzbromarone.

 

Drug Interactions

 

Allomaron increases the toxicity of azathioprine and mercaptopurine, the anticoagulant effect of coumarin derivatives, lengthens the action of chlorpropamide.

 

Thiazide diuretics, derivatives of ethacrylic and salicylic acids reduce the effect of Allomaron (increase the level of uric acid in the blood).

 

When treatment with iron increases the accumulation of iron in the liver.
