Orthoflex - non-steroidal anti-inflammatory drug (NSAID), a derivative of phenylacetic acid. Has a pronounced anti-inflammatory, analgesic and moderate antipyretic effect.

The mechanism of action is associated with inhibition of cyclooxygenase (COX) activity, the main enzyme of arachidonic acid metabolism, which is a precursor of prostaglandins, which play a major role in the pathogenesis of inflammation, pain and fever. The analgesic effect is due to two mechanisms: peripheral (indirectly, through suppression of prostaglandin synthesis) and central (by inhibiting the synthesis of prostaglandins in the central and peripheral nervous system). Inhibits the synthesis of proteoglycan in the cartilage.

When rheumatic diseases reduces pain in joints at rest and during movement, as well as morning stiffness and swelling of the joints, contributes to an increase in the volume of movements. Reduces post-traumatic and post-operative pain, as well as inflammatory edema.

Composition

Diclofenac sodium + auxiliary substances.

Pharmacokinetics

After oral administration, it is absorbed (absorbed) from the gastrointestinal tract (GIT). Eating slows the rate of absorption. About half of the active substance is metabolized by the "first pass" through the liver. With rectal administration, absorption is slower. The time to reach the maximum concentration in the plasma after ingestion is 2-4 hours, depending on the dosage form used, after rectal administration - 1 hour, and after intramuscular injection - 20 minutes. Does not accumulate in the body. Binding to plasma proteins is 99.7% (mainly with albumin). It penetrates into the synovial fluid, where the maximum concentration of the drug substance is reached 2-4 hours later than in the plasma. It is metabolized to a great extent with the formation of several metabolites, among which two are pharmacologically active, but to a lesser extent than diclofenac. Approximately 60% of the dose is excreted as metabolites by the kidneys, less than 1% - with urine unchanged, the rest - in the form of metabolites with bile.

Response of a rheumatologist

I prescribe Ortoflex ointment to patients with joint pain, both for the first time seeking help, and for those who have long suffered from gout or rheumatoid arthritis. Patients already after the first day of using the ointment note its good analgesic effect. In addition, patients say that the ointment is consumed very sparingly. If you smear 2-3 times a day, one large joint, for example, the knee, then a tube of ointment (30 g) lasts almost a week. Adverse reactions to this drug in the form of skin rashes, itching or burning of the skin in my practice was not one of the patients. Only once the patient with inflammation of the elbow joint smeared the skin over him with two ointments containing diclofenac. And on the same day she addressed me with complaints about the bright reddening of the skin in the area of the elbow. I advised her to take a shower, to wash off the remnants of ointments well and continue to use only one Orthoflex. After this, the patient no longer had such problems.
