Medovir - instructions for use, reviews, analogs and forms of release (powder for the preparation of a solution for intravenous droppers 250 mg) drugs for the treatment of various forms of herpes, chicken pox, immunodeficiency in adults, children, in pregnancy

In this article, you can read the instructions for using the drug Medovir. Presented are reviews of visitors to the site - consumers of this medication, as well as opinions of medical experts on the use of Medovir in their practice. A big request is to actively add their feedback on the drug: the medicine helped or did not help get rid of the disease, which were observed complications and side effects, possibly not declared by the manufacturer in the annotation. Analogues of Medovir in the presence of existing structural analogues.Use for the treatment of herpetic infection, simple and herpes zoster, chicken pox, immunodeficiency in adults, children, as well as during pregnancy and lactation.

Medovir antiviral agent. Thymidine kinase virus-infected cells actively convert Acyclovir (the active ingredient of Medovir) through a series of consecutive reactions to the mono-, di- and triphosphate of acyclovir. The latter is embedded in the deoxyribonucleic acid (DNA) synthesized for new viruses, interacting with the viral DNA polymerase. Thus, "defective" viral DNA is formed, which leads to suppression of replication of new generations of viruses.

Acyclovir is active against the Herpes simplex virus types 1 and 2, the Varicella zoster virus, the Epstein-Barr virus and cytomegalovirus.

Composition

Acyclovir (in the form of sodium salt) + auxiliary substances.

Pharmacokinetics

When administered, the bioavailability is 15-30%. Widely distributed in tissues and body fluids. Binding to plasma proteins is 9-33%. Metabolised in the liver. It is excreted in the urine, in a small amount - with feces.

Indications

  • infections caused by viruses Herpes simplex types 1 and 2 and Varicella zoster: shingles, chicken pox and others;
  • prevention of infections caused by the viruses Herpes simplex and Varicella zoster (including in patients with reduced immunity);
  • pronounced immunodeficiency (including in the clinical picture of HIV infection) as part of complex therapy;
  • condition after bone marrow transplantation;
  • prevention of cytomegalovirus infection after bone marrow transplantation.

Forms of release

Powder for the preparation of a solution for intravenous administration of 250 mg (for intravenous drippers).

Instructions for use and dosing regimen

Intravenously drip adults and children over 12 years of age - 5-10 mg per kilogram of body weight, the interval between administrations is 8 hours. Children aged 3 months to 12 years - 250-500 mg per square meter of body surface, the interval between administrations is 8 hours. For newborns, the dose is 10 mg per kilogram of body weight, the interval between administrations is 8 hours.

In case of renal failure, correction of the dosing regimen is necessary.

Maximum doses: for adults with intravenous administration - 30 mg per kilogram of body weight per day.

Side effect

  • acute renal insufficiency;
  • crystalluria (crystals of salts in urine);
  • encephalopathy (confusion, hallucinations, agitation, tremors, convulsions, psychosis, lethargy, coma);
  • phlebitis (inflammation of veins) or inflammation at the site of intravenous administration;
  • nausea, vomiting.

Contraindications

  • increased sensitivity to acyclovir and valacyclovir;
  • lactation (breastfeeding).

Application in pregnancy and lactation

The use of Medovir in pregnancy is possible in cases where the intended use for the mother exceeds the potential risk to the fetus.

Intravenous use of acyclovir during the lactation period is contraindicated (excreted in breast milk).

In experimental animal studies, it has been shown that acyclovir penetrates the placental barrier.

Use in children

Intravenously drip to children older than 12 years - 5-10 mg per kilogram of body weight, the interval between administrations is 8 hours. Children aged 3 months to 12 years - 250-500 mg per square meter of body surface, the interval between administrations is 8 hours. For newborns, the dose is 10 mg per kilogram of body weight, the interval between administrations is 8 hours.

Application in elderly patients

Treatment of elderly patients should be carried out with sufficient increase in water load and under the supervision of a physician, since in this category of patients the half-life of acyclovir is increased.

special instructions

It is not recommended for use in severe renal dysfunction.

It should be taken into account that the use of acyclovir may lead to acute renal failure due to the formation of a precipitate from acyclovir crystals, which is especially likely with rapid intravenous administration, simultaneous use of nephrotoxic drugs, in patients with impaired renal function and under inadequate water load.

When using Medovir, it is necessary to monitor the kidney function (determining the level of urea Nitrogen in the blood and creatinine in the blood plasma).

In the treatment of genital herpes, sexual intercourse should be avoided or condoms should be used. the use of acyclovir does not prevent the transmission of the virus to partners.

Drug Interactions

With the simultaneous use of probenecid reduces the tubular secretion of acyclovir and thereby increases the concentration in the blood plasma and the half-life of acyclovir.

When used simultaneously with mycophenolate, mycophenolate increases the pharmacokinetic parameter (AUC), which characterizes the total concentration and acyclovir, and the inactive metabolite of mycophenolate mofetil in the blood plasma during the entire observation time.

With the simultaneous use of acyclovir with nephrotoxic drugs, the risk of developing nephrotoxic action increases (especially in patients with impaired renal function).

When mixing solutions, it is necessary to take into account the alkaline reaction of acyclovir for intravenous administration (pH 11).

Response of infectious diseases physician

In my practice, I sometimes use Medovir to treat herpes infection. Patients with severe, poorly amenable therapy in outpatient settings, forms of shingles and chicken pox are often admitted to our infectious disease department. And this is not only adult patients, but also children of different ages. The intravenous drip introduction of Medovir in such situations is justified, firstly, by the severity of the condition of the patients. Secondly, this drug is much less likely to cause adverse reactions compared to other antiviral agents containing acyclovir and available in tablets, ointments, creams.
