A big request is to actively add their feedback on the drug: the medicine helped or did not help get rid of the disease, which were observed complications and side effects, possibly not declared by the manufacturer in the annotation. Analogues of Clopixol in the presence of existing structural analogues.Use for the treatment of schizophrenia, psychosis in adults, children, as well as in pregnancy and lactation. Composition of the preparation.

Clopixol - antipsychotic agent (neuroleptic), thioxanthene derivative. It is believed that the antipsychotic effect of zuclopentixol (the active substance of Clopixol) is due to the blockade of dopamine receptors in the central nervous system. Thioxanthene derivatives have a high affinity for dopamine D1 and D2 receptors.

Zuclopentixol causes a fast, transient, dose-dependent sedative effect, before the development of antipsychotic action.

Unlike zuclopenthixol hydrochloride, zuclopenthixol acetate has a longer duration of action - 2-3 days, and zuclopenthixola decanoate is a depot form and its effect persists for 2-4 weeks.

Composition

Zuclopentixola dihydrochloride + auxiliary substances.

Pharmacokinetics

After oral administration, zuclopenthixol is rapidly absorbed from the digestive tract. Cmax in blood plasma is achieved after 3-6 hours. After intramuscular injection as depot form, Cmax in the blood plasma is reached after 36 hours. 3 days after the injection, the level is approximately 1/3 of the maximum.Zuclopentixol is distributed in the body, with higher concentrations created in the liver, lungs, intestines and kidneys, lower - in the heart, spleen, brain and blood. Binding to plasma proteins 98%. Penetrates through the placental barrier, excreted in breast milk. Zuclopentixol is metabolized by sulfoxidation, N-dealkylation and glucuronization. Metabolites do not have psychopharmacological activity. It is excreted mainly with feces in the form of unchanged substance and N-dealkylated metabolite.

Indications

For oral administration:

  • manic phase of manic-depressive psychosis;
  • mental retardation in combination with psychomotor agitation, agitation and other behavioral disorders;
  • senile dementia with paranoid ideas, confusion, disorientation, behavioral disorders.

For the / m introduction:

  • initial treatment of acute psychoses, manic states and chronic psychoses in the phase of exacerbation.

For the / m introduction of the depot form:

  • Supportive therapy for schizophrenia and paranoid psychosis.

Forms of release

Instructions for use and effective doses of the drug

Inside. Doses are selected individually, depending on the patient's condition; begin usually with small doses, which are quickly increased to achieve the optimal effect.

In case of exacerbation of schizophrenia, other acute psychiatric disorders, severe agitation, mania usually 10-50 mg per day, with pronounced disorders and moderate states usually begin with 20 mg per day, then, if necessary, increase the dose by 10-20 mg every 2-3 days to 75 mg per day or more.

In chronic mental disorders (schizophrenia and other psychoses) in a maintenance dose of 20-40 mg per day.

When agitating in patients with oligophrenia - usually 6-20 mg per day; if necessary, increase the dose to 25-40 mg per day.

With agitation and confusion in patients with senile disorders - usually 2-6 mg per day (preferably in the evening); if necessary, 10-20 mg per day.

Side effect

  • dizziness;
  • drowsiness (especially when using the drug in high doses and at the beginning of treatment);
  • development of extrapyramidal disorders (mainly at the beginning of treatment);
  • violation of accommodation;
  • tardive dyskinesia;
  • tachycardia;
  • orthostatic hypotension;
  • dry mouth;
  • constipation;
  • minor transient changes in hepatic samples;
  • retention of urine.

Contraindications

  • acute overdose of barbiturates, opioid receptor agonists;
  • acute alcohol intoxication;
  • coma;
  • increased sensitivity to zuclopentixol.

Application in pregnancy and lactation

The use of Clopixol during pregnancy and lactation (breast-feeding) is not recommended.

Zuclopentixol in low concentrations is found in breast milk.

Use in children

Not noted.

special instructions

Clopixol is not used in patients with intolerance of neuroleptics for oral administration, as well as in patients with Parkinson's disease.

With extreme caution apply in patients with convulsive syndrome, chronic hepatitis and cardiovascular diseases.

With prolonged use, careful monitoring of the patient's condition is necessary.

It should be borne in mind that with the use of neuroleptics, a rare but possible complication with a fatal outcome is the development of malignant neuroleptic syndrome (CNS). In such cases, the use of neuroleptics should be stopped immediately and emergency symptomatic therapy started.

In the period of Clopixol treatment, antiparkinsonian drugs should be prescribed only if there are appropriate indications and do not apply prophylactically.

Clopixol should not be used concurrently with guanethidine and other agents with a similar mechanism of action.

Impact on the ability to drive vehicles and manage mechanisms

During the treatment with zuclopentixol, especially in the beginning, it is necessary to avoid activities requiring high concentration of attention and rapid psychomotor reactions.

Drug Interactions

With simultaneous use with drugs that exert a depressing effect on the central nervous system, with ethanol (alcohol), narcosis drugs intensify their depressing effect on the central nervous system, the effect of the means for anesthesia is potentiated.

With simultaneous application with guanethidine, it is possible to reduce or completely suppress the hypotensive effect of guanethidine.

A case of the development of the NSA is described with simultaneous application with clorazepate.

With simultaneous application, it is possible to reduce the effects of levodopa and adrenergic agents.

With the simultaneous use of lithium carbonate may increase the side effects of Clopixol.

With simultaneous use with metoclopramide, Piperazine increases the risk of developing extrapyramidal symptoms.
