In this article, you can read the instructions for using the drug Moditen. There are reviews of visitors to the site - consumers of this medication, as well as the opinions of the doctors of specialists on the use of Moditen in their practice. A big request is to actively add their feedback on the drug: the medicine helped or did not help get rid of the disease, which were observed complications and side effects, possibly not declared by the manufacturer in the annotation. Analogues Modiena with existing structural analogues. Use for the treatment of schizophrenia, psychosis in adults, children, as well as in pregnancy and lactation. Composition of the preparation.

Moditen - antipsychotic agent (neuroleptic), Piperazine derivative of phenothiazine. Has a pronounced antipsychotic effect, combined with some activating effect. It is believed that the antipsychotic effect of phenothiazines is due to the blockade of postsynaptic dopamine receptors in the mesolimbic structures of the brain. Has a weak antiemetic effect, hypotensive effect, has some anticholinergic activity. Antiemetic action is due to blockade of dopamine D2 receptors in the chemoreceptor trigger zone of the cerebellum, as well as blockade of the vagus nerve in the digestive tract. The hypotensive effect is associated with blockade of alpha-adrenergic receptors, inhibition of the release of hormones of the hypothalamus and pituitary gland. However, blockade of dopamine receptors increases the prolapse of the pituitary gland. Fluphenazine also has an extrapyramidal effect.

Composition

Fluphenazine decanoate + excipients.

Pharmacokinetics

Clinical data on the pharmacokinetics of fluphenazine are limited. Phenothiazines have a high binding to plasma proteins. They are mainly excreted by the kidneys and partly by bile.

Indications

  • psychotic disorders, incl.schizophrenia;
  • states accompanied by hyperactivity and excitement;
  • anxious states, severe psychomotor agitation, agitation, dangerous impulsive behavior (associated with violent actions);
  • psychoses, especially paranoid;
  • manic and hypomanic states;
  • supporting the treatment of chronic schizophrenia without the phenomena of agitation after reaching a stable state with the help of neuroleptics of a shorter action (for depot form).

Forms of release

Tablets 1 mg, 2.5 mg and 5 mg.

Solution for intramuscular administration of oily Moditen Depot (injections in ampoules for injection).

Instructions for use and dosing regimen

Pills

Inside, with anxiety and other nonpsychotic behavior disorders - 1 mg 2 times a day, if necessary - 2 mg 2 times a day; with schizophrenia and other psychoses, hypo- and manic states - 2.5-10 mg (up to 20 mg) per day in 2 divided doses; aged - in doses corresponding to the lower limits of the indicated doses.

Ampoules

Intramuscularly deep, adults in the initial dose of 25-100 mg (elderly patients 12.5-50 mg), maintaining a dose of 12.5-25 mg (elderly - 12.5 mg), the interval between injections is 15-35 days.Single dose should not exceed 100 mg. If it is necessary to use in a dose exceeding 50 mg, the initial dose for each subsequent administration is increased by 12.5 mg.

Intramuscularly or subcutaneously, fluphenazine decanoate is used as a depot form in children aged 5-12 years - 3.125-12.5 mg every 1-3 weeks, 12 years and older - 6.25-25 mg every 1-3 weeks.

Application in pregnancy and lactation

Safety of Modity during pregnancy is not established, therefore, if necessary, care should be carefully weighed against the expected benefit to the mother and the risk to the fetus.

If it is necessary to use lactation, breastfeeding should be discontinued.

Use in children

Fluphenazine is not recommended for use in children.

Intramuscularly or subcutaneously, fluphenazine decanoate is used as a depot form in children aged 5-12 years - 3.125-12.5 mg every 1-3 weeks, 12 years and older - 6.25-25 mg every 1-3 weeks.

Application in elderly patients

Use with caution in elderly patients.

special instructions

With caution apply for liver diseases, arrhythmias, thyrotoxicosis, epilepsy, Parkinson's disease, zakratougolnoy glaucoma, with myasthenia; patients working in high temperature conditions who have contact with phosphorus-containing insecticides; in elderly patients, as well as with increased sensitivity to other phenothiazines.

In the case of hyperthermia, which is one of the symptoms of a malignant neuroleptic syndrome (NSH), Moditen should be immediately withdrawn.

Simultaneous use of phenothiazines with adsorbent antidiarrhoeic agents should be avoided.

During the treatment period, do not drink alcohol.

Impact on the ability to drive vehicles and manage mechanisms

During the treatment period, it is necessary to refrain from potentially dangerous activities requiring increased attention, rapid psychomotor reactions.

Drug Interactions

With the simultaneous use of drugs that have a depressing effect on the central nervous system, ethanol (alcohol), ethanol-containing drugs may increase the depression of the central nervous system and respiration.

With simultaneous use with anticonvulsants, the threshold of convulsive readiness may be reduced.

With the simultaneous use of drugs to treat hyperthyroidism, the risk of developing agranulocytosis increases.

With the simultaneous use of drugs that have anticholinergic effects, it is possible to increase their anticholinergic action, and the antipsychotic effect of the antipsychotic may decrease.

When used simultaneously with drugs that cause extrapyramidal reactions,possibly increased frequency and severity of extrapyramidal disorders; with agents that cause arterial hypotension, pronounced orthostatic hypotension is possible; with tricyclic antidepressants, maprotiline, MAO inhibitors - it is possible to increase the risk of ZNS.

With simultaneous use, it is possible to suppress the effects of amphetamines, levodopa, clonidine, guanethidine, adrenaline.

With simultaneous use with antacids, the absorption of Modityen may be impaired.

The case of the development of neurotoxicity symptoms (tremor, rigidity of muscles, ataxia, weakness, vomiting, confusion) is described with simultaneous application of lithium carbonate.

With simultaneous application with ephedrine, the vasoconstrictive effect of Ephedrine may be weakened.
