Composition of the preparation.

Lidevin - A combined drug for the treatment of alcohol dependence, is a combination of Disulfiram with group B vitamins.

Disulfiram is an inhibitor of acetaldehyde dehydrogenase. Blocking of acetaldehyde dehydrogenase leads to an increase in the concentration of acetaldehyde, which is a metabolite of ethyl alcohol.Acetaldehyde causes negative feelings (including flushing, nausea, vomiting, tachycardia, low blood pressure), which make it extremely unpleasant alcohol after taking Lidevina. This leads to a conditioned reflex aversion to the taste and smell of alcoholic beverages.

The maximum therapeutic effect is achieved 12 hours after ingestion and can last for 10-14 days after discontinuation of treatment.

The combination of disulfiram with vitamin B3 (niacinamide) and B4 (adenine) provides pronounced clinical effect upon receipt of even small doses of alcohol, has minimal side effects, reduces the toxicity of disulfiram ensures prevention of alcoholic neuropathy and hypovitaminosis.

Composition

Disulfiram + Nicotinamide (vitamin B3) + Adenine (vitamin B4) + auxiliary substances.

Pharmacokinetics

After oral absorption of Lidevin from the gastrointestinal tract is 70-90%. Disulfiram is rapidly metabolized, until recovering dithiocarbamate which is output as a glucuronate conjugate or turns in diethylamine and carbon sulphide, part (4-53%) is output through the lungs.

After 7-10 days of therapy, a disulfiramalcoholic sample is taken (20-30 ml of 40% vodka after taking 500 mg of the drug), with a weak reaction, the dose of alcohol is increased by 10-20 ml (the maximum dose is 100-120 ml). The sample is repeated after 1-2 days in the hospital and 3-5 days after an outpatient treatment, if necessary - with the correction of the doses of alcohol and / or the drug. Drug Interactions

Combinations are contraindicated

With the simultaneous reception with alcohol, the reaction of intolerance develops (hot flushes, erythema, vomiting, tachycardia). Avoid taking drinks and medicines containing ethanol.

Unwanted combinations

With simultaneous application of Lidevin and isoniazid, there may be a violation of behavior and coordination.

With simultaneous use with drugs from the group of derivatives of nitro-5-imidazole (metronidazole, ornidazole, seksnidazol, tinidazol), it is possible to develop delirious disorders, confusion.

With the simultaneous application of phenytoin, the development of toxic effects of phenytoin (due to a slowing of its metabolism and an increase in the concentration in the blood plasma) is possible.

Combinations that require caution

With the simultaneous use of Lidevin and Warfarin (or other oral anticoagulants), the risk of bleeding increases because of the increased effect of warfarin (slowing the biotransformation of the latter).

Lidevin slows down the biotransformation of theophylline, therefore, with simultaneous use, a dose reduction of theophylline is required depending on the clinical symptoms and its concentration in the blood plasma.

With the simultaneous use of Lidevin potentiates the sedative effect of benzodiazepines by inhibiting the oxidative pathway of metabolism of the latter, especially chlordiazepoxide and diazepam. The dose of benzodiazepine should be adjusted in accordance with clinical manifestations.

With the simultaneous use of Lidevin and tricyclic antidepressants, it is possible to intensify the reactions of alcohol intolerance, especially if patients take alcohol while taking Lidevin.
