Analgin - An analgesic-antipyretic. Metamizole sodium (the active substance of the preparation Analgin) is a derivative of pyrazolone and has an analgesic, antipyretic and weak anti-inflammatory effect, the mechanism of which is associated with inhibition of prostaglandin synthesis by inhibition of COX.
Analgin is banned in most countries of Western Europe, as an anesthetic and antipyretic agent due to the threat of agranulocytosis and the main consumer of this drug recently is Russia and the CIS countries.
Pharmacokinetics
After oral administration, metamizole sodium is rapidly hydrolyzed in gastric juice with the formation of an active metabolite of 4-methyl-amino-antipyrine, which, after absorption, is metabolized to 4-formylamino-antipyrine and other metabolites. After intravenous administration metamizole sodium quickly becomes inaccessible for determination. Metabolites of metamizole sodium do not bind to plasma proteins.Most of the dose is excreted in the urine in the form of metabolites. Metabolites are excreted in breast milk.
Indications
- pain syndrome of different etiology (including headache, neuralgia, arthralgia, myalgia, myositis, algodismenorea, postoperative pain);
- renal and biliary colic (in combination with antispasmodics (but the spine or drotaverin));
- feverish conditions with infectious and inflammatory diseases.

Application in pregnancy and lactation
Use with caution in pregnancy, especially in the first trimester and in the last 6 weeks.
special instructions
With prolonged use, it is necessary to monitor the picture of peripheral blood.
Metamizole sodium is used in combination with pitofenone and fenpiverinia bromide as an analgesic with an antispasmodic effect.
Use with caution in children in the first 3 months of life.
Drug Interactions
With simultaneous use with analgesics-antipyretics, with NSAIDs, mutual enhancement of toxic effects is possible.
With simultaneous use with inductors of microsomal liver enzymes, a decrease in the efficacy of metamizole sodium is possible.
With simultaneous use, the activity of indirect anticoagulants, oral hypoglycemic drugs, GCS, Indomethacin is increased as a result of their displacement from the connection with blood proteins under the influence of metamizole sodium.
With simultaneous use with phenothiazine derivatives, pronounced hyperthermia is possible; with sedatives, anxiolytics - the analgesic effect of metamizole sodium is enhanced; with tricyclic antidepressants, oral contraceptives, Allopurinol - metabolism of metamizole sodium is disrupted and its toxicity is increased; with Caffeine - increases the effect of metamizole sodium; with cyclosporine - the concentration of cyclosporine in the blood plasma decreases.
When metamizole sodium is used in combination with pituophenone hydrochloride (it has a direct myotropic effect on the smooth muscles of the internal organs and causes it to relax) and with fenpiverinium bromide (m-cholinoblocker), their pharmacological effect is mutually reinforced, which is accompanied by a reduction in pain syndrome, relaxation of smooth muscles and a decrease in fever.
