A big request is to actively add their feedback on the drug: the medicine helped or did not help get rid of the disease, which were observed complications and side effects, possibly not declared by the manufacturer in the annotation. Analogues of the antibiotic Flagil in the presence of existing structural analogues.Use for the treatment of trichomoniasis, giardiasis and other infections in adults, children, as well as during pregnancy and lactation. Composition and interaction of the drug with alcohol.

Flagel refers to nitro-5-imidazoles. The mechanism of action of Metronidazole (the active substance of Flagil) is the biochemical reduction of the 5-nitro group of metronidazole by intracellular transport proteins of anaerobic microorganisms and protozoa. The restored 5-nitro group of metronidazole interacts with the DNA of a cell of microorganisms, inhibiting the synthesis of their nucleic acids, which leads to the death of microorganisms. The main metabolite (2-oxymetronidazole) also has antiprotozoal and antimicrobial effects.

Metronidazole is an effective antimicrobial and antiprotozoal means of a wide spectrum of action. The drug exhibits high activity with respect to Trichomonas vaginalis, Giardia intestinalis, Entamoeba histolytica, Lamblia intestinalis (lamblia), and also to obligate anaerobes (sporo- and non-spore forming) - Bacteroides spp. (B. fragilis, B.ovatus, B.distasonis, B.thetaiotaomicron, B.vulgatus), Fusobacterium spp., Clostridium spp., Peptostreptococcus spp., Peptococcus spp., Sensitive strains of Eubacterium.

Metronidazole is not sensitive to aerobic microorganisms and facultative anaerobes.

Composition

Metronidazole + auxiliary substances.

Pharmacokinetics

When ingested Flagyl quickly and almost completely absorbed (approximately 80% per hour). Eating does not affect the absorption of the drug. Bioavailability is approximately 100%. The drug quickly penetrates into tissues (lungs, kidneys, liver, skin, bile, spinal fluid, saliva, seminal fluid, vaginal secret), into breast milk and passes through the placental barrier. Metabolized about 30-60% of metronidazole by hydroxylation, oxidation and glucuronidation. The main metabolite (2-oxymetronidazole) also has antiprotozoal and antimicrobial effects. Removal of the drug is carried out at 40-70% through the kidneys (in unchanged form - about 35% of the dose).

Indications

  • protozoal infections: extraintestinal infections, including amoebic liver abscess, intestinal amoebiasis (amoebic dysentery), trichomoniasis, balantidiasis, giardiasis, cutaneous leishmaniasis, trichomonas vaginitis, trichomoniasis urethritis;
  • infections caused by Bacteroides spp. (including Bacteroides fragilis, Bacteroides distasonis, Bacteroides ovatus, Bacteroides thetaiotaomicron, Bacteroides vulgatus): infections of bones and joints, infections of the central nervous system, incl. meningitis, brain abscess, bacterial endocarditis, pneumonia, empyema and lung abscess, sepsis;
  • infections caused by species of Clostridium spp.,Peptococcus and Peptostreptococcus: infections of the abdominal cavity (peritonitis, liver abscess), infections of the pelvic organs (endometritis, abscess of the fallopian tubes and ovaries, vaginal vaginal infection);
  • pseudomembranous colitis (associated with the use of antibiotics);

Instructions for use and dosage

Pills

Inside. With amebiasis, Flagyl is administered inside for 7 days to 1.5 g per day for 3 doses (adults).

Giardiasis is treated for 5 days: adults are prescribed 750-1000 mg per day, children 6-10 years old - 375 mg per day (1.5 tablets) and children 10-15 years - 500 mg per day (2 tablets). For children under 6 years of age, it is preferable to use metronidazole in the form of a suspension.

With trichomoniasis in women (urethritis and vaginitis), Flagil is administered once in a dose of 2 g or as a course of treatment for 10 days: 1 tablet (250 mg) 2 times a day.

For trichomoniasis in men (urethritis), Flagil is given once in a dose of 2 g or as a course treatment for 10 days, 1 tablet (250 mg) 2 times a day.

With nonspecific vaginitis, 500 mg Flagyl is used 2 times a day for 7 days.

In the treatment of anaerobic infections, adults are prescribed Flagil at 1-1.5 g per day.

At the expressed infringements of function of kidneys (KK less than 10 ml / min) it is recommended to reduce a daily dose of a preparation in 2 times.

Candles for vaginal

The drug is used only in adults.

Trichomonas vaginitis: 1 suppository per day in the vagina, for 10 days in combination with taking the drug metronidazole, tablets inside.

Nonspecific vaginitis: 1 suppository 2 times a day for 7 days, if necessary in combination with taking the drug metronidazole, tablets inside. It is necessary to simultaneously treat the sexual partner with metronidazole, regardless of whether he has manifestations of the disease.

Treatment with metronidazole should not be prescribed for more than 10 days, and should be repeated more than 2-3 times per year.

Preliminarily releasing the suppository from the contour package using scissors, cut the film along the contour of the suppository and insert it deep into the vagina.

special instructions

Since simultaneous administration of the drug with alcohol can have an effect similar to the effect of Disulfiram (a feeling of heat, vomiting, tachycardia), patients should be warned that during treatment Flagil should not drink alcohol or drugs containing alcohol.

Long-term administration of the drug is desirable under the control of the blood formula. With leukopenia, the possibility of continuing treatment depends on the risk of developing an infectious process.

When treating trichomonadal vaginitis in women and trichomoniasis in men, it is necessary to abstain from sexual activity. It is necessary to simultaneously treat sexual partners. Treatment does not stop during menstruation. After therapy with trichomoniasis, control tests should be performed for 3 consecutive cycles before and after menstruation.

After treatment of giardiasis, if symptoms persist, after 3 to 4 weeks, 3 feces should be analyzed at intervals of several days (in some successfully treated patients lactose intolerance caused by invasion,can persist for several weeks or months, recalling the symptoms of giardiasis).

It should stop treatment when ataxia, dizziness, the development of hallucinations, with worsening neurological status of patients.

It must be taken into account that metronidazole can immobilize treponema, which leads to Nelson's false positive test.

Impact on the ability to drive vehicles and manage mechanisms

In connection with the possibility of dizziness and other side effects associated with taking the drug, it is recommended to refrain from driving and other mechanisms.

Drug Interactions

Similarly, disulfiram causes intolerance to ethanol (alcohol).

Simultaneous use with disulfiram can lead to the development of various neurologic symptoms (the interval between appointments is at least 2 weeks).

Strengthens the effect of indirect, anticoagulants, which leads to an increase in the time of formation of prothrombin.

With simultaneous administration with lithium preparations, the concentration of the latter in plasma can increase.

With simultaneous administration with cyclosporine, the concentration of the latter in the plasma may increase.

Cimetidine suppresses the metabolism of metronidazole, which can lead to an increase in its concentration in the blood serum and an increased risk of side effects.

Simultaneous administration of drugs that stimulate enzymes of microsomal oxidation in the liver (phenobarbital, phenytoin), can accelerate the elimination of metronidazole, as a result of which its concentration in the plasma decreases.

Metronidazole reduces the clearance of 5-fluorouracil, leading to an increase in its toxicity.

Metronidazole increases the level of busulfan in the blood plasma, leading to an increase in its toxicity (severe form).

It is not recommended to combine with nondepolarizing muscle relaxants (vecuronium bromide).

Site visitor question my mother moved to the required section:
Hello, tell or say, please, and at the dairy flagil helps or assists. This is also metronidazole, too.

my mother, Yes, Flagil contains Metronidazole and is an analogue of this drug. And with regard to thrush or candidiasis, the flag that Metronidazole are largely useless, since the spectrum of antimicrobial activity does not affect the fungi Candida that cause candidiasis.
